Complete the Medical Benefits Application (VA Form 10-10 EZ) and bring it to the Patient Enrollment Office located on the East Orange VA Medical Center Campus.
What to bring with your health care application
For a faster decision on your application, please bring the following documents and information:
- Copy of DD214 or other discharge documents
- A copy of any VA disability award letter you may have
- Earnings information for the previous year if VA disability rate is 0% or if you are a non-service veteran
- A copy of your current ID card from any other type of health insurance (such as Medicare, Medicaid, TRICARE, or private health insurance)
